Photo ID or care card, your insurance or CDCP information, a list of any medications, and previous X-rays if you have them. No referral needed.
We accept most dental insurance plans and the Canadian Dental Care Plan — including for children — and we bill your insurance directly — every plan we accept — so you pay only your portion at the visit.
